Financial Aid Checklist

Get help paying for college!  Our checklist will guide you through the process.

 

  1. Complete your Federal taxes.
    Your spouse (if applicable) and/or parents' taxes may also be required. http://www.irs.gov/.
  2. Complete the Free Application for Federal Student Aid (FAFSA).
    In order to be considered for the most desirable aid, please make sure your file is complete by April 1.
  3. Search and apply for scholarships.
  4. Complete the University of Utah Online Financial Aid application (UFORM).
  5. If requested, complete the Verification form.
    If you are classified as a dependent, please submit the Dependent Verification Form and a signed legible copy of yours and your parent(s) Federal taxes, all schedules, and W-2s.  If you are classified as Independent, please submit the Independent Verification Form, yours and your spouse's (if applicable) Federal taxes, all schedules, and W-2s. 
  6. Check your Financial Aid Status in Campus Information Services.CIS
  7. Accept your Financial Aid online in Campus Information Services.CIS
  8. If accepting loans, complete the Loan Counseling for Stafford and/or Loan Counseling for Perkins Loans.
    This must be done every year you accept a loan.
  9. Complete Master Promissory Notes (MPN), if necessary
         The MPN is a legally binding agreement between you and us and contains the terms and conditions of your loans.  (Students borrowing with a Perkins Loan must complete a Promissory Note each year.)
