Your - Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification. The law says that before awarding Federal Student Aid, we may ask you to confirm the information you and your spouse, if married reported on your FAFSA. To verify that you provided correct information the financial aid administrator at your school will compare your FAFSA with the information on this worksheet and with any other required documents. If there are differences, your FAFSA information may need to be corrected. You and at least one parent must complete and sign this worksheet, attach any required documents, and submit the form and other required documents to the financial aid administrator at your school. Your school may ask for additional information. If you have questions about verification, contact your financial aid administrator as soon as possible so that your financial aid will not be delayed.

How to complete the form successfully:

  1. Complete the student information section.
  2. Scroll down and complete the demographic information tab. Please use a valid e-mail address for the form. The University of Utah has partnered with DocuSign in order to allow you to electronically sign your verification worksheet. You will receive an email from DocuSign once the form is ready to be signed. If you do not want to electronically sign the form, please contact our office for a paper copy.
  3. Use the next button and complete the household information tab.
  4. Use the next button and complete the income information tab.
  5. Click the submit button to submit the information. If you need to change any information, click the edit button.

Indicate the student's marital status.

List below the people in the student's household. Include:
  • The student.
  • The student's spouse, if the student is married.
  • The student's or spouse's children if the student or spouse will provide more than half of the child's support from July 1, , through June 30, , even if a child does not live with the student.
  • Other people if they now live with the student and the student or spouse provides more than half of the other person's support and will continue to provide more than half of that person's support through June 30, .
 Note: We may require additional documentation if we have reason to believe that the information regarding the household members enrolled in eligible postsecondary educational institutions is inaccurate.

Student Income Information

Did the student or spouse, if applicable, file a  income tax return with the IRS?

Important Note: The instructions below apply to the student and spouse, if the student is married. Notify the financial aid office if the student or spouse filed separate IRS income tax returns for  or had a change in marital status after December 31, .

Instructions: Complete this section if the student and spouse filed or will file a  IRS income tax return(s). The best way to verify income is by using the IRS Data Retrieval Tool (IRS DRT) that is part of FAFSA on the Web at FAFSA.gov. In most cases, no further documentation is needed to verify  income information that was transferred into the student's FAFSA using the IRS DRT if that information was not changed by the FAFSA filer.

  • Get Transcript ONLINE - Go to www.IRS.gov, under the Tools heading on the IRS homepage, click "Get a Tax Transcript." Click "Get Transcript ONLINE." Make sure to request the "IRS Tax Return Transcript" and NOT the "IRS Tax Account Transcript."
  • Get Transcript by MAIL - Go to www.IRS.gov, under the Tools heading on the IRS homepage, click "Get a Tax Transcript." Click "Get Transcript by MAIL." Make sure to request the "IRS Tax Return Transcript" and NOT the "IRS Tax Account Transcript."
  • Automated Telephone Request - 1-800-908-9946
  • Paper Request Form - IRS Form 4506T-EZ or IRS Form 4506-T

In most cases, for electronic tax return filers,  IRS income tax return information is available for the IRS DRT or the IRS Tax Return Transcript within 2-3 weeks after the  electronic IRS income tax return has been accepted by the IRS. Generally, for filers of  paper IRS income tax returns, the  IRS income tax return information is available for the IRS DRT or the IRS Tax Return Transcript within 6-8 weeks after the  paper IRS income tax return has been received by the IRS. Contact the financial aid office if more information is needed about using the IRS DRT or obtaining an IRS Tax Return Transcript.

If the student and spouse filed separate  IRS income tax returns, the IRS DRT cannot be used and the  IRS Tax Return Transcript(s) must be provided for each

Please do not send any Tax related forms to our office or the University via email. We are unable to accept tax documents sent through email. You may mail, fax or bring the documents to our office.

 Additional documentation will be required for those who did not file taxes. Please complete a - Independent Student Non-Filer statement at http://financialaid.utah.edu/forms/
